Review the Elevated MD terms of service governing access to our website, telehealth platform and administrative wellness-program services.
Acceptance and eligibility
By using this website, you agree to these terms and linked policies. You must be at least 18 years old and legally able to enter an agreement. Services are available only where permitted and may be limited by jurisdiction.
Platform role
Elevated MD supports technology, administration, payment and care coordination. Independent licensed medical providers make clinical decisions. Licensed pharmacies dispense and ship prescription medication when a valid prescription has been issued. Elevated MD does not practice medicine and does not itself prescribe or dispense medication.
Not emergency care
The services are not for emergencies. Call 911 or seek immediate in-person care for a medical emergency. Online content is not a substitute for advice from your healthcare professional.
Program purchases and prescriptions
A purchase may cover administrative services, technology access, provider evaluation, follow-up, pharmacy coordination and other stated program components. It does not guarantee a prescription. You agree to provide complete and accurate health, identity, shipping and payment information.
Pricing and recurring programs
Current prices, program durations, inclusions and any recurring billing terms are presented before checkout. Taxes and other disclosed charges may apply. Where recurring billing is selected, you authorize charges on the disclosed schedule until the commitment ends or cancellation takes effect under the applicable policy.
Communications
You consent to operational email, telephone and text communications needed to administer your request. Marketing consent is not a condition of purchase, and available opt-out instructions will be provided. Do not use email for emergencies or sensitive medical details.
Acceptable use and intellectual property
You may not misuse the site, interfere with its security, submit unlawful content or copy protected materials except as permitted. Site content and branding belong to Elevated MD or their respective licensors.
Disclaimers and limitation
Services are provided subject to applicable law and without promises of specific clinical or commercial results. To the fullest extent permitted by law, liability is limited to direct damages associated with the applicable transaction. Some jurisdictions do not permit certain limitations.
